In the lore, Adam Smasher is a nearly unkillable death machine (in the stories in Cyberpunk RED anyway) where he is only stopped by a nuke going off while fighting Morgan Blackhand (who is suspected to have lived this encounter). So Blackhand couldn't kill him, and a nuke couldn't kill him.
